Documents necessary to lodge a passport application:
1. Fully completed Passport Application Form (www.passports.gov.au)
2. 2 Photographs, one signed on the reverse side by guarantor
3. Previous Australian passport (if applicable)
4 Original birth certificate (if not in English, official translation or international birth certificate is required).
5. Certificate of Australian Citizenship (original), if born outside Australia.
6. Evidence of change of name (marriage certificate, deed poll, decree nisi, etc.) with an Apostille if event happened outside of Australia.
7. Photo ID (e.g. foreign passport, driver's licence)
8. Additional proof of identification (e.g. bank card, social insurance card, health card etc.)
